# Add Spouse/Dependent

> Add a spouse or dependent to your existing insurance policy

## Steps to Add a Dependent

1. **Complete Health Application**
   * Download the [health application form](/forms/health-application)
   * Or complete the [health application online](/forms/online-application)
   * Submit through your customer portal

2. **Prepare Required Documentation**
   * Dependent's personal information
   * Passport copy
   * Proof of relationship (marriage certificate, birth certificate, etc.)
   * Current visa/residence permit (if applicable)
   * Completed health application

3. **Submit Your Request**
   Submit all documentation through your customer portal. Our team will review and process your request within 5-10 business days.
